    •  ACT takes a holistic approach to treatment, addressing not only the individual's mental health symptoms but also their social, vocational, and housing needs to support overall well-being and recovery.

  • Personalized Meeting Locations

    •  Treatment may take place in the client’s home, workplace, or out in the community. This approach, allows our team to meet clients where they are, providing support and interventions in familiar environments where clients feel most comfortable and where their challenges may be most apparent

Adult Community Services

The Community Support Services (CSS) program provides a broad array of services and supports in the community for adults who live in Brown, Doniphan, Jackson, or Nemaha counties whose mental health challenges cause moderate to severe disruption to daily life activities and relationships. CSS services are provided where a person lives, works, goes to school, or socializes. To be eligible for these services, individuals must have an evaluation by a therapist to ensure that they meet criteria. 

 

Additional ACT Services

ACT (Assertive Community Treatment) serves a select group of adult clients aged 18 and above diagnosed with serious mental illness (SMI) or substance use disorders. These individuals experience severe symptoms that significantly impede their ability to live independently within the community. ACT aims to provide comprehensive, community-based support for individuals with severe mental illnesses or substance use disorders. It's designed to offer intensive, individualized services to help people manage their symptoms and live successfully in their communities.

Targeted Case Management

Designed to provide comprehensive support and guidance to young people facing mental health challenges. Our team of dedicated professionals specializes in working with youth and their families to navigate the complexities of the mental health care system and access the resources they need to thrive. Through personalized care plans and advocacy, we help youth and their families connect with community resources, access treatment services, and develop the skills necessary for success. ​
