Assertive Community Treatment

Assertive Community Treatment (ACT) is a specialized, community-based service that provides comprehensive support for individuals living with severe mental health conditions and/or substance use disorders. ACT is designed to help people achieve greater independence, stability, and quality of life while remaining in their own homes and communities.

Unlike traditional office-based services, ACT brings support directly to the individual. A team of trained professionals works closely with each person to provide personalized care based on their unique needs, goals, and strengths. Services are available in real-life settings, such as at home, work, school, or other community locations, making support more accessible and effective.

  • Housing and independent living skills

ACT focuses on helping individuals reduce hospitalizations, improve overall well-being, and develop the skills needed to live successfully in the community. By providing ongoing, coordinated, and person-centered care, ACT empowers individuals to work toward recovery, achieve personal goals, and lead meaningful lives.

Additional ACT Services

ACT (Assertive Community Treatment) serves a select group of adult clients aged 18 and above diagnosed with serious mental illness (SMI) or substance use disorders. These individuals experience severe symptoms that significantly impede their ability to live independently within the community. ACT aims to provide comprehensive, community-based support for individuals with severe mental illnesses or substance use disorders. It's designed to offer intensive, individualized services to help people manage their symptoms and live successfully in their communities.

Counseling

✔ Crisis Intervention
Immediate support and assistance for individuals experiencing a mental health crisis, helping to ensure safety, stabilization, and connection to appropriate resources.​

✔ Comprehensive Assessment
A thorough evaluation of mental health, substance use, and overall needs to help determine the most appropriate level of care and treatment recommendations.

✔ Substance Use Treatment
Evidence-based services designed to help individuals address substance use concerns, develop recovery skills, prevent relapse, and improve overall well-being.

✔ Individual Supportive Therapy
One-on-one counseling focused on emotional support, coping skill development, problem-solving, personal growth, and achievement of treatment goals.

✔ Case Management
Coordination of care and community resources, including assistance with healthcare, benefits, housing, employment, transportation, and other supportive services to promote stability and independence.
